You must fill out the form completely.

We must have your order number.

We must have your item description as most orders have multiple items.

Please upload a picture of your picture guide to illustrate the colors missing and packets. (This is MANDATORY)

We can not process incomplete orders.

REQUESTS FOR DIAMONDS MUST BE MADE WITHIN 10 DAYS OF RECEIVING YOUR ITEM.

REQUESTS SUBMITTED AFTER THAT WILL BE IGNORED.
